There are no required forms for enrolling in the university’s 401(a) Plan. Complete your enrollment online.

To enroll in the university’s 403(b) Plan, you must complete a Salary Reduction Agreement (SRA) Form in order to process your enrollment application. An SRA allows automated, pre-tax contributions from your paycheck directly to your specified plans. You can obtain a copy of these forms at your benefits office. SRA’s must be received by university CU Employee Services by the 10th day of the month in order to defer salary into the 403(b) Plan at that month’s end.


Have the following information available before you enroll online:
  • Your investment allocation amounts/percentages for your investment choices.
  • Your Social Security number.
  • Your beneficiary's Social Security number, birth date and address.
