  • Your investment allocation amounts/percentages for your investment choices.
  • Your Social Security number.
  • Your beneficiary's Social Security number, birth date and address.

You may select either of the first 2 plans, or choose to enroll in both plans concurrently by selecting the third link.

REQUIRED FORM

Employees are required to complete a Salary Reduction Agreement form to initiate the payroll contributions from their paychecks. Please contact the Benefits Office at retirement@temple.edu or call 215 926-2281 if you need this form.
