REQUIRED FORM

You are required to complete a Salary Reduction Agreement (SRA) for one or more of your plans. An SRA allows automated, pre-tax contributions from your paycheck directly to your specified plans.

Complete a Salary Reduction Agreement for the Tax-Deferred Annuity Plan (Group Supplemental) (PDF).

Submit the form(s) to your Benefits Office.

ENROLL ONLINE

Have the following information available before you enroll online:

Your investment allocation amounts/percentages for your investment choices.
Your Social Security number.
Your beneficiary's Social Security number, birth date and address.

Defined Contribution Plan (Employer Contributions) NOTE: Enrollment in the TU Employer Plan is available for employees who have completed two years of service with TU. For details consult with the Human Resources Benefits Office.
Tax-Deferred Annuity Plan (Group Supplemental) Voluntary Elective Deferral Plan. Enrollment available on a voluntary basis for all employees. For details consult with the Human Resources Benefits Office.
