You are required to complete a Salary Reduction Agreement (SRA) for one or more of your plans. An SRA allows automated, pre-tax contributions from your paycheck directly to your specified plans.

Submit the form(s) to your Benefits Office.


Have the following information available before you enroll online:

  • Your investment allocation amounts/percentages for your investment choices.
  • Your Social Security number.
  • Your beneficiary's Social Security number, birth date and address.

  1. Download your enrollment materials from the appropriate link(s) below:
    • Complete and sign the forms.
    • Mail the forms to:

When enrolling in the West Virginia Community and Technical Colleges 403(b) Supplemental Plan and/or the 457(b) Deferred Compensation Plan, you must fill out the Salary Reduction Agreement in addition to the Enrollment Form. Please note: Brokerage is limited to Mutual Funds only in the 403(b) Supplemental Plan, but there are no limitations for brokerage in the 457(b) Deferred Compensation Plan.
